目的:探讨吸气肌联合膈肌抗阻训练对脑卒中患者呼吸功能的疗效。方法:选取2018年12月至2020年4月山东中医药大学附属医院康复科收治的脑卒中患者88例,采用随机数字表法分为呼吸训练组和对照组各44例。两组均根据患者具体功能障碍状况给予针对性常规康复训练,呼吸训练组在此基础上给予吸气肌联合膈肌抗阻训练,每日1次,治疗4周。评估患者吸气肌肌力、肺通气功能的改善状况。结果:在康复训练中共出现7例失访,总体依从率92.05%。康复训练后,呼吸训练组吸气峰流速、1 s用力呼气量(FEV1)、1 s用力呼气量/用力呼气肺活量(FEV1/FVC)、每分钟最大通气量(MVV)4个指标较训练前均显著改善,且呼吸训练组显著高于对照组,差异均具有统计学意义(P<0.05)。康复训练后,两组吸气肌肌力指数均有不同程度的提高,呼吸训练组患者的吸气肌肌力指数评分显著高于对照组,差异具有统计学意义(P<0.05)。结论:吸气肌联合膈肌抗阻训练能够显著改善脑卒中患者的呼吸肌肌力及肺通气功能。
Abstract:Objective:To investigate the effect of inspiratory muscle training combined with diaphragmatic resistance training on pulmonary ventilation function of stroke patients. Methods:Eighty-eight stroke patients admitted to the Rehabilitation Department of The Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December2018 to April 2020 were included. All patients were randomly divided into the respiratory training group and the control group according to the random number table method. Both groups were given specified routine rehabilitation training according to the dysfunction status of patients,while the respiratory training group was given inspiratory muscle combined with diaphragm resistance training besides the treatment in the control group,once a day for four weeks. The improvement of inspiratory muscle strength and pulmonary ventilation function was evaluated. Results:There were 7 cases of drop-out,and the overall compliance rate was 92.05%. After rehabilitation treatment,four indexes,including inspiratory peak velocity,forced expiratory volume in the 1st second(FEV1),FEV1/FVC and maximal ventilatory(MVV) volume per minute in the respiratory training group were significantly improved compared with those before intervention(P<0.05),and the indexes in the respiratory training group were significantly higher than those in the control group and the differences were statistically significant(P<0.05). After rehabilitation treatment,and the mspiratory muscle strength indexes of the two groups increased to different degress,the inspiratory muscle strength index score of the respiratory training group was significantly higher than that of the control group and the differences were statistically significant(P<0.05). Conclusion:Inspiratory muscle training combined with diaphragmatic resistance training could significantly improve inspiratory muscle strength and pulmonary ventilation function of stroke patients.
