目的:应用中医临床科研一体化平台,研究张素芳教授推拿治疗小儿泄泻的选穴特点。方法:收集整理张教授2014年至2016年门诊治疗的小儿泄泻病案,运用Excel软件建立病案数据库并进行频数分析,应用Liquorice软件制作复杂网络图,分别对该病的症状、治疗处方以及五个证型的核心处方进行分析。结果:共统计分析病案615例,涉及推拿处方操作115个,通过频数统计和复杂网络研究,得到该病及寒湿泻、湿热泻、伤食泻、脾虚泻、脾肾阳虚泻五个证型的核心处方共6首,得出了疾病-症状、疾病-证型、疾病-穴位、症状-穴位之间的关系。结论:张教授推拿治疗小儿泄泻,重在健脾和胃、化湿行气、清肠止泻,调理脾、胃、大肠的功能,治疗时遵循辨病选穴、对症选穴、辨证选穴原则。核心处方为:补脾经、掐揉四横纹、清大肠、按揉脾俞、按揉胃俞、按揉大肠俞。并针对各证型辨证加减取穴与采用不同手法。
Abstract：Objective:To study the acupoints selection regularity of Professor ZHANG Sufang in massage therapy for treatment of infantile diarrhea by using Traditional Chinese Medicine(TCM) Clinical Research Integration Platform. Methods:The infantile diarrhea cases treated by Professor ZHANG Sufang from 2014 to 2016 at clinics were collected and organized. Excel software was used to build a medical record database and perform frequency analysis. The Liquorice software was used to make complex network diagrams to analyze the symptoms,treatment prescriptions and core prescriptions of the five syndromes respectively.Results:A total of 615 cases were statistically analyzed,involving 115 manipulation of massage prescription. Through frequency statistics and complex network research,six core prescriptions of this disease and the five syndromes of diarrhea caused by cold and dampness,heat and dampness,food accumulation,spleen deficiency and spleen and kidney yang deficiency were obtained,and the relationship between disease-symptom,disease-syndrome,disease-acupoint and symptom-acupoint was obtained. Conclusion:In the massage therapy to treat infantile diarrhea,Professor ZHANG Sufang focuses on strengthening the spleen and stomach,removing dampness and promoting the circulation of qi,clearing the intestines and stopping diarrhea,and regulating the functions of the spleen,stomach and large intestine. She follows the rules of selecting acupoints according to the disease,symptom and syndrome. The core prescriptions are:reinforcing the spleen,pinching and rubbing the four horizontal lines,clearing the large intestine,pressing and rubbing the spleen Shu,pressing and rubbing the stomach Shu,pressing and rubbing the large intestine Shu. According to the syndrome differentiation,the acupoints are selected and different manipulations are used.
